I need you to convert my customer list into a specific CSV format for importing into FlowTrack Pro, a backflow testing management app.

OUTPUT FORMAT — use these exact column headers:
First Name,Last Name,Company Name,Phone,Email,Address,City,State,ZIP,Location Name,Location Address,Location City,Location State,Location ZIP,Location Contact,Location Phone,Location Notes

RULES:
1. DEDUPLICATE COMPANIES: If multiple rows belong to the same company (e.g. "G & M Oil - #3", "G & M Oil - #5", "G & M Oil - #12"), consolidate them into ONE customer with the parent company name (e.g. "G & M Oil Company"). Each individual site becomes a separate row with the same company info but different Location fields.

2. CUSTOMER vs LOCATION:
   - First Name, Last Name, Company Name, Phone, Email = the BILLING/PARENT entity (same across all locations for that company)
   - Address, City, State, ZIP = the billing/main address
   - Location Name = a short label for each service site (e.g. "Station #3 - La Habra", "Building A")
   - Location Address, City, State, ZIP = the physical SERVICE address where backflow devices are located

3. PARSE COMBINED FIELDS: If city, state, and ZIP are combined in one field like "Huntington Beach, CA 92647" or "La Habra, CA. 90631", split them into separate City, State, and ZIP columns.

4. CLEAN UP:
   - Remove periods from state abbreviations (CA. -> CA)
   - Standardize phone numbers
   - If First Name / Last Name are empty but there's a contact name, use that
   - Skip any rows with no useful address or company info
   - If a row has a billing address but no service/ship-to address, leave the Location fields blank

5. ONE ROW PER LOCATION: A company with 5 locations = 5 rows, all with the same customer info but different Location columns.

6. Output as CSV text I can paste into a spreadsheet. No extra commentary, just the CSV data.
